State Athletic Commission of California Report of Physical Examination, listing Dempsey’s legal name, “William Harrison Dempsey,” followed by his ring name, “Jack Dempsey.” Report details his medical history and the current examination, including his height, weight, pulse, blood pressure, vision, reflexes, etc. Signed at the conclusion by Dempsey and the attending physician. In very good condition, with four horizontal folds, surface and corner creasing, a few scattered edge tears, one repaired with clear tape at the top edge, a staple mark to each top corner, two punch holes to the top edge, and a office “received” stamp. An uncommon report brimming with Dempsey’s personal information, who, at 63 years old, still found his way into the ring.
