^Lot 1236 Ernie Kovacs

Kovac’s rare yellow AFTRA union P&W benefits card, one page, both sides, 6 x 4, dated September 20, 1959. Pension and Welfare benefit card is filled out in type including his California address, birthdate and Social Security number. Signed at the bottom in blue ballpoint “Ernie Kovacs.” On the reverse in type Kovac’s beneficiaries including all of his children and wife are listed. Written across the front of the card is “Deceased, 1/13/62.” Several office notations and home address crossed out and written anew, and a small bit of paper loss to top edge, otherwise fine condition.
