American actress, socialite, fashion model and heiress (1943–1971). Sedgwick is best known for being one of Andy Warhol's superstars. Sedgwick’s rare yellow AFTRA union P&W benefits card, one page, both sides, 6 x 4, dated Sept 3, 1965. Pension and Welfare benefit card is filled out by Sedgwick in black ink, printing her full name, “Edith Minturn Sedgwick,” her New York City address, birthdate and Social Security number. Signed at the bottom “Edith M. Sedgwick.” On the reverse, Sedgwick lists her mother, Mrs. F. M. Sedgwick, as the recipient of her death benefits. Unobtrusive green pencil notation, otherwise fine condition.
