^Lot 1545 Vince Lombardi

Lombardi’s rare yellow AFTRA union P&W benefits card, one page, both sides, 6 x 4, dated September 23, 1965. Pension and Welfare benefit card is filled out in type including his full name, Wisconsin address, birthdate and Social Security number. Signed at the bottom in blue ballpoint “Vincent Lombardi.” On the reverse in type is listed his wife, Marie, as the recipient of his death benefits. Stamped across the front of the card is “Deceased.” A few small office notations and a vertical crease to middle of card otherwise fine condition.
