Lot 1396 Oscar Levant

Pianist, radio personality and author, perhaps best remembered for his autobiography, Memoirs of an Amnesiac. Levant’s rare yellow AFTRA union P&W benefits card, one page, both sides, 6 x 4, dated November 1, 1963. Pension and Welfare benefit card is filled out (possibly by Levant), with his Beverly Hills address, birthdate, and Social Security number. Signed at the bottom in black ballpoint “Oscar Levant.” On the reverse, his wife is listed as the recipient of his death benefits. Stamped across the front of the card is “Deceased.” Moderate vertical crease down middle of document, several office notations, and a staple hole to top left, otherwise fine condition—the signature area remains clean.
